h + s policy ?

Been asked to provide a health and safety policy / statement ive never had to write one before anyone done one what has to be in it how long does it have to be or has anyone got a template they can email me

also been asked for coshh declaration form um a what ?

I dont think you need a h + s policy unless you employe more than 5 people
